HOW TO REFER

Three steps, no forms

There is no referral form to chase and no portal to log into. An email or a phone call is enough — we will come back with questions if we need them.

Step one

Send what you have

Email or call with the participant’s name, funding type and what you need. A two-line summary is plenty. If you have a plan or previous report, attach it; if not, we will work without it.

Same business day

We come back to you

Capacity confirmed, timing proposed, and a report date if an assessment is involved. If we cannot take the referral we say so immediately and suggest who might.

From there

We take it from here

We book directly with the participant or family, deliver the service, and share reports and progress with your team — with consent. You are not chased for scheduling.

  • Access notes — interpreter, animals, best times

One question saves the most time: how is the plan managed? We work with plan-managed and self-managed participants, so knowing that at referral means we never assess someone we cannot invoice — and you never wait on an answer we could have given on day one.

NDIS

Plan & self managed

Billed under the current NDIS Pricing Schedule. Both Improved Daily Living (0128) and Early Childhood (0118) items.

Aged care

Support at Home

Available as an associated provider or directly to self-managed clients. Travel is built into the rate, as the scheme requires.

Private

No referral needed

Clients can book directly. Medicare rebates apply where a GP has written a Chronic Disease Management plan.

QUESTIONS

What referrers usually ask

The same business day, every time. If a referral arrives late in the afternoon you will hear from us before we finish, not the following morning. We would rather tell you we are full than leave you waiting.
Not currently. We work with plan-managed and self-managed NDIS participants. Telling us the management type at referral means we never assess someone we cannot invoice — and you get a straight answer immediately.
Yes, with consent. Reports and progress go to whoever the participant nominates, and we close the loop when a piece of work is delivered rather than leaving you to chase it.
Yes. Children under 9 are supported through the early childhood approach and billed against the 0118 items. We work with families, early childhood partners and schools.
We are taking new clients across all regions at the time of writing. Capacity is stated on this page and in the referrer packs, and we update it rather than leaving it optimistic.
For NDIS, travel is charged separately in line with the Pricing Schedule and agreed in the service agreement. For aged care, travel must be built into the hourly rate and is never invoiced separately. Full detail is in our fee schedule.
